NEW 11/19: Ordering H1N1 2009 Monovalent Vaccine
Targeted sub-groups for priority vaccination have been expanded to include all persons aged 19-64 who have underlying medical conditions that put them at higher risk for influenza-related complications. To order within the current priority groups being targeted, you must order by noon, Thursday, to receive H1N1 vaccine the next week using this form/instructions.
The same form is due each Thursday at noon for the following week's vaccine order, with the exception of Nov. 26 (Thanksgiving).
NOTE:  There are eight different formulations of vaccine. DPH receives a daily allotment of vaccine. Vaccine providers are encouraged to order multiple formulations and not limit orders to preservative-free vaccine, which is in very short supply. If you limit your order to one formulation (e.g. preservative-free, pre-filled syringes), your order may go unfilled. It is unlikely that any order will be completely filled in any given week.

If your primary care physician is not offering vaccine, you and your staff can contact your local Mass Dispensing Area to be put on a list for priority vaccination.
